Specialist Support Coordination

Level 3 Specialist Support Coordination

Specialist Support Coordination supports participants with highly complex needs, increased risk, or involvement across multiple systems who require advanced coordination and advocacy.

Big Ability provides Level 3 Specialist Support Coordination for participants with complex Autism, neurodiverse profiles, and neurological or progressive conditions, particularly where there are challenging behaviours, unstable supports, safeguarding concerns, hospital admissions or discharges, or repeated service breakdowns.

We work closely with NDIS, DFFH housing, hospitals, health services, and other state-based stakeholders to deliver structured, NDIA-aligned responses. Our focus is on stabilising supports, reducing risk, and maintaining continuity of care, while guiding participants and families through complex systems with clarity and confidence.

Support may include:

  • Crisis support and hospital discharge coordination

  • Multi-agency collaboration and state service liaison

  • Housing pathways, including DFFH, SIL, SDA, and Home and Living

  • Behavioural and environmental support coordination

  • Safeguarding, risk management, and escalation processes

  • Reviews, changes of circumstances, and evidence coordination

This service is suited to participants who need specialist system navigation, ongoing oversight, and strong advocacy to achieve safe and sustainable outcomes.

Support Coordination

Level 2 – Support Coordination

Support Coordination assists participants to implement their NDIS plan, build capacity, and develop the skills needed to manage supports more independently over time.

Big Ability provides Level 2 Support Coordination for participants who need guidance to set up, coordinate, and maintain supports, without the complexity or risk factors that require specialist intervention. We work alongside participants and families to translate NDIS plans into practical, workable supports that align with individual goals.

Our focus is on education, empowerment, and proactive coordination, helping participants understand their plan, build strong provider relationships, and reduce reliance on crisis responses.

Support may include:

  • Implementing and coordinating NDIS-funded supports

  • Connecting with suitable providers and services

  • Supporting decision-making, planning, and problem-solving

  • Monitoring supports and addressing issues early

  • Assisting with plan reviews and goal progression

This service is best suited to participants who want practical guidance, structured support, and increased independence in managing their NDIS supports.

SIL/SDA Housing

SIL :

Supported Independent Living (SIL)
Supported Independent Living assists participants to live as independently as possible, whether in shared or group accommodation, a private rental, with family, or in their own home. SIL focuses on daily living support tailored to individual needs and goals

SDA :

Specialist Disability Accommodation (SDA)
Specialist Disability Accommodation refers to housing designed for participants with extreme functional impairment or very high support needs, where specialist design features and ongoing supports are required.

Where to Start ?

SIL/SDA applications require the completion of Home and Living Application Form.
The SIL and SDA application will also require engagement and in depth reports with your allied health- Occupational therapist/ Physiotherapist in order to help complete assessments required within the Home and Living Form available on the NDIS website.

Aged Care

Expert Guidance

The National Disability Insurance Act 2013 provides guidance around what will and won’t fund. For participants residing within Aged Care facilities there needs to be expert and hands on guidance on navigating both.

Younger People in Residential Aged Care (YIPRAC)

For participants under 65 years of age on the NDIS, the Australian government recognises that the aged care system is not designed to provide support to younger people under the age of 65. The YIPRAC team sits internally within NDIS and is designed to help assist the governments pledge to reduce the amount of younger people residing in aged care facilities.

Transitioning from Aged Care

Transitioning from Aged Care to suitable accommodation requires expert guidance, assistance with helping find and secure the right accommodation to best suit the care needs of the participant and family.
Big Ability provides assistance in liaising with the suitable Supported Independent living (SIL) providers, Supported Disability Accommodation (SDA) applications along with collaboration with the YIPRAC team to successfully transition to suitable accommodation.

NDIS Reviews and Appeals

Sometimes an NDIS plan no longer reflects a person’s needs. When this happens, a plan reassessment or appeal may be required. Recent legislative changes mean there are now specific rules about what the NDIA will reassess, and when and how changes can be requested.

Big Ability provides expert guidance through each stage of the review and appeals process, ensuring requests are evidence-based, timely, and NDIA-aligned.

Change of Circumstances / Situation

A Change of Circumstances is used to notify the NDIA of significant changes such as:

  • Living arrangements

  • Functional capacity

  • Informal supports or carers

  • Support needs or risk factors

While there are no strict time limits, requests must be supported by clear, relevant evidence demonstrating how circumstances have changed.

Review of a Reviewable Decision (S100)

If you receive an NDIS decision you don’t agree with, you may request an internal review of a reviewable decision (S100). This is often the first formal step in challenging an NDIA decision

Administration Reviews Tribunal (ART)

Administrative Review Tribunal (ART)

The Administrative Review Tribunal (ART) (formerly the AAT) is an independent body that can review most internal NDIA review decisions under the NDIS Act 2013.

If you have received a decision you don’t agree with, the ART have the ability to review most internal review decisions made by the NDIA under the National Disability Insurance Scheme Act 2013 (NDIS act)
